When a patient presents to the birthcenter with say for instance, braxton-hicks contractions or feared spontanous rupture of membranes, and is sent home after evaluation, is it appropriate for our physicans to bill and E&M level outside of the global package?

I would say no. It's part of the global package if it's related to the pregnancy. You can track the visit, and some payors will allow you to carve the visit out if the total # of visits exceed 13 by the time of delivery, and the patient care had no interruptions with outside docs or insurance changes. I've been billing OB for the past 15 years, so I know the OB docs are hassling you over this one :)

Thanks for your reply Heidi. It's nice to get feedback. It's such a "gray" area of coding that's its good to hear other coders points of view.
