Many times a physician will make this statement meaning the patient has lung cancer which has metastasized(never could spell that word), The 1985 May/June coding clinic states to treat this statement as Lung cancer primary(162.9) with unknown secondary(199.1), the rationale is that lung is far more common as a primary site than it is as a secondary site. If you remeber that the metastatic site is still the primary just located in a different organ, so bone mets could be stated as lung cancer located in the bone.
