Patient was in a MVA and has a large open wound of the nose. There are septal and nasal fractures. Nasal septum was straigtened thru the open wound and also the nasal pyramid bones straightened and stabilzed through the soft tissue envelope on both the left and right sides.
My main question is that can these septal and nasal fracture be coded with the open treatment codes? 21325 and 21336 are the codes I am considering. Per CPT instructions for open treatment is used when the fxd bone is either: surgically opened to expose the fx or fx'd bone is opened remote from fracture site. But what happens if this presents as a open wound, can the open treatment codes still be used?
