would you code a dx. of p/o breast cancer with v10.3. history of malignant neoplasm. (breast) also if pt had a double masstectomy and has to have surgery again, would her pre-op clearence be coded as:

consult 99244
dx V72.83, 174.9 (malignant neoplasm of female breast) or would you use the V10.3 for history of breast cancer
