PA's and CRNP's new vs. est

I have recently started billing for a new Urologist. I am receiving claims back from Medicare stating that the patients are established based on a PA seeing a patient. The PA works for Internal Medicine. How do I make this a new patient for the Urologist? The PA does not work for the same specialist.
