Wiki Patient inpatient at rehab but received outpatient surgery

I am having issues getting a claim paid through horizon, the patient was inpatient at a rehab facility, but was at a hospital outpatient center for resection of intracranial reasons with stereotactic surgery. They are denying for POS conflicting with another claim on file,etc.We are not at all affiliated with the facility she was admitted to. Our office is the providers who performed the surgery.

Any articles, tips, etc would be greatly appreciated.
