Pharmacy Managing the IV Vancomycin

This is a question for hospital E/M's. When a doctor decides to start a patient on IV Vanc., this counts as drug therapy requiring intensive monitoring for toxicity. I have been giving them credit for their initial decision to start it, but I'm a little confused on whether they should keep getting credit on all of the subsequent visits. In our hospital, the pharmacy is consulted to monitor/dose the Vanc. Should the physician still get credit for drug therapy requiring intensive monitoring for toxicity on subsequent visits, even though the pharmacist is doing the monitoring? I feel like they shouldn't.
