If a surgeon specifies in his pre-op order the he is requesting a peripheral nerve block for post-op pain mgmnt, is this acceptable documentation per guidelines?

Are you asking if you can bill the peripheral nerve block? I would say it has to be stated it was performed to be billed.
I am doing a post bill audit. The documentation used to support the nerve block are the signed physician's order and the anesthesiologist's procedure note performing the block. The surgeon does not document in his operative report that the nerve block was for post operative pain.
