Physical Therapy rules for medicare every 10th visit.

I am a new biller working in a new PT office and had a question about medicare billing. Does my PT need to do a re-eval on a pt every 10 visits? if so how do code this for medicare. I am not sure how to go about this. Any info will be helpful thank you.
