I have a physician that would like to bill a z-plasty closure (14000) with the excision of pilonidal cyst (11772) I don't feel that it is appropriate to bill then together since the CCI coding companion states "closure of the defect is included in this code, and should not be reported separately". There are not any CCI edits though, so the Dr. thinks they should be billed. Does anyone have any experience with this? Any and all help would be appreciated.

Read below, description includes Z-plasty if necessary:

In 11772, the sinus involves many subcutaneous extensions superficial to the fascia overlying the sacrum. The physician uses a scalpel to completely excise the cystic tissue. Local soft tissue flaps (i.e., Z-plasty, Y-V plasty, myofasciocutaneous flap) may be required for closure of a large defect or the wound may be left open to heal by granulation
