Wiki PPD Administration prior to biologics

We have patients that will be going on biologic medications and need to have a tuberculosis screening prior to starting. I am using the CPT code 86580 and an administration code 90772 and a nurse visit for the return reading 99211, but what diagnosis code would I use?

Am I coding this correctly? Any help is appreciated.
