Sometimes the ones that seem so easy I cant seem to figure out. My Dr. does this procedure alot on the common femoral, but this was in the ankle and he did the tibial artery. Any ideas?



PREOPERATIVE DIAGNOSIS: Right foot pseudoaneurysm of the right anterior tibial artery.

POSTOPERATIVE DIAGNOSIS: Right foot pseudoaneurysm of the right anterior tibial artery.

OPERATIVE PROCEDURE: Repair of right forefoot pseudoaneurysm off the anterior tibial artery.

ANESTHESIA: General.

ESTIMATED BLOOD LOSS: Minimal.

SPONGE AND INSTRUMENT COUNTS: Correct.

DESCRIPTION OF OPERATION AND FINDINGS: After proper consent was obtained and placed in the chart, patient was transported to the operative theater by department of anesthesia who was present throughout the case for general sedation. Right foot was sterilized and prepped in the usual fashion. A 15 blade scalpel was used to make an incision to expose the anterior tibial artery, at which time proximal and distal control was obtained. Patient was administered 8000 units of intravenous heparin. After 3 minutes of time, clamps were placed. A large pseudoaneurysm was identified as to the anterior tibial artery and a small defect in the arterial wall. This was repaired with 6-0 Prolene suture. Clamps removed. The pseudoaneurysm was evacuated of its previous wall and thrombus material. The site was copiously irrigated and suctioned. With hemostasis assured, the incision site was closed in a multilayer fashion.

The patient tolerated the procedure well. Was taken to recovery in stable condition.
