I am confused which I should code? I am thinking coding the supra tear would be more specific then rotator cuff, so I am leaning towards that?
 
Concur with True-always verify first with the op note. See post-op diagnosis, but verify that the actual work was done on this tendon by reviewing the body of the report.
If I am charge posting or if I am coding this out pre-op to schedule surgery
I also like to review the MRI report to verify the exact damaged tendon, as there are many possibilities in the shoulder and upper arm. The surgeons like to just say "rotator cuff tear", but it's important to be as specific as possible, especially with ICD-10 coming up.
