ICD-9-CM 2009 coding guidelines page 25 section I Uncertain diagnosis:
Do not code diagnoses documented as "probable", "suspected", "questionable", "rule out", or "working diagnosis" or other similar terms indicating uncertainty. Rather, code the condition(s) to the highest degree of certainty for that encounter/visit, such as symptoms, signs, abnormal test results, or other reason for the visit. Please note: this differs from teh coding practices used by short-term, acute care, long-term care and psychiatric hospitals.
So, according to the coding guidelines, you should not code a rule out diagnosis. I would get further clarification. Why did they come in? Dizziness? Diaphoresis? There has to be some reason they suspect they are hypoglycemic? That's what I would code.
