In order to bill for splint application, is it required that the splint be created by the physician or can you bill splint application when using pre-fab splints? I am working with a client that has typically given away velcro type splints and has never billed for applying these to their patients since they did not "create" the splint in their office.
 
Does this also apply to items like wrist, ankle, knee braces and air casts that are pre-fabricated? Also if the supply is included in the procedure, what about casting materials? I thought those were separately billable from the procedure.
