Wiki Split/Shared Billint

Is the NP and physician seeing the patient together on the same visit? Is the physician the "supervising physician" signing off on the NP's progress note?

No, the physician is not always the supervising physician but they are part of the same group. They are rounding together but the NP is doing the progress note.
If you're using both provider's work to determine the level of the visit this should be billed as a split/shared visit under the NPI of the provider who performed the "substantive portion" of the visit.

If the physician just signs off on the PA's note, the claim goes under the PA's npi number. Does the modifier -FS still need to be applied?

If the physician only signed the note, but did not provide part of the service, it is not a split/shared visit and -FS would not apply.
