Hello, I am new to Surgery Coding, and I need some clarification. Ok, here's 3 scenarios: Surgeon performs a knee replacement. Do I just list the procedure w/out a modifier? Next scenario: Surgeon performs a procedure w/Assist from PA, do I list the procedure twice with the AS modifier on the PA? Next scenario: Surgeon perfoms a procedure w/Assist from another MD, do I list the procedure twice with the 80 modifier on the assisting MD? Any help would be greatly appreciated.
 
1) lt or rt would be appropriate for surgeon alone
2) AS is appropriate if the PA has his own NPI number and they would be listed on two separate claims as long it is not a teaching facility
3) 80 is appropriate if it is not a teaching facility if it is then 82 would be most appropriate.

I would also include the lt or rt on the assist's billing.
