If a benign or malignant lesion is removed from the front or back of the ear, the decision may be made to allow the wound to heal by secondary intention. That normally requires surgical fenestration in one and up to multiple sites to facilitate granulation tissue coverage of the exposed cartilage surface. Would it be appropriate in this scenario to use the unspecified '99'. Sometimes 6-8 individual full thickness excisions are made through the entire thickness of the ear cartilage under local anesthetic in the skin on both the front and the back of the ear. It seems like a lot of work to do to not get paid for doing it.
