I am wondering if there is a HCPCS or CPT code to bill when a physician reviews a Tandem 365 form?
TANDEM365 is a collaborative venture assisting hospitals, doctors, family members and individuals in need of healthcare services not traditionally provided elsewhere. They are not bound by rules of a Home Health Agency and basically the PCP is reviewing orders to help keep patient's out of the emergency room and hospital. Evidently the Tandem365 does bill the patient's insurance for this service but not sure if we as the PCP can bill and if so what code is it? Here is a little more information: The organization coordinates medical, behavioral health and social services for older people who generally have multiple chronic conditions such as congestive heart failure, COPD, emphysema, dementia, kidney failure, Parkinson’s and diabetes. Oftentimes the patients are in frail health, lack a social support network and need daily or weekly assistance at home.
