Wiki Tee and Cardiac Catherizations

I have a physician who bills for the Tee with his cardiac catherizations. we are having trouble getting the Tee paid. I am also having trouble locating a clear answer. We billed the following codes
93312 26
93320 26
93325 26
93454 26
93455 26 59
93463 59
93458 26 59
93456 26 59

Should I be billing the Tee with a modifier 59 as well, or is the Tee considered to be part of the catherization?

Are you billing all those Heart cath codes together on the same ticket or just giving an example of the heart cath codes you are billing with the tee?

You can not bill all of the heart cath codes together. Can you post a report to double check your codes?
