Pt. had a metal-on-metal THA done several years ago. Now having pain.

The metal ball was removed, but repeated attempts to remove the liner were unsuccesful. There was no wear of the liner. Therefore, the metal ball was replaced with a ceramic ball. Nothing else was done. What is the proper revision code?

I would look at 27138 (Femoral component?) and add a 52 mod for reduced services. Just ask your doc about the work entailed in the ball/head replacement. Maybe you don't need 52 mod.
