Toe nail trimming/koh/wet mount

Should I be using the G0127 for a medicare patient that comes in for their nail trimmings instead of 11719? Also I have a medicare patient that came in for abnormal vaginal bleeding and the provider did a wet mount and koh. Should I use the Q0111 and Q0112 instead of the 87210 and 87205?

In order for this service to be covered you must use the appropriate Class Findings Modifier and there must be a systemic condition as the secondary diagnosis. You also must have the physician treating the systemic condition and the date last see by that physician in the appropriate spot or the claim will deny.
