A locum tenens is filling in for a physician out for surgery. The facility also does urine drug screens for multiple locations of the same company and the rendering (box 24J) has always been listed as the physician there in the location where he sees patients in the building with the urine drug screen analyzer. Can I submit claims to payers with the locum tenens in 24J if the provider out for surgery goes off payroll and goes on long term disability?

You use the original providers name, but add a Q6 modifier on each line to be billed. This tells the insurance co. that it is a locum in place of a regular provider. This is for the first 60 days. After the 60 days, the new provider must be credentialed with your company.
