Hello, a little insight needed.

Patient developed postoperative thrombosis after DIEP.

Procedure 1 : Reoperation and exploration for postoperative thrombosis, chest. (35820 -78)

Procedure 2: 13 cm of saphenous vein harvested from left leg with 3 segmental incisions in order to graft and repair venous system at a separate site on chest. (??)

SO, a 3.0 mm coupler was used to anastomose the deep inferior epigastric vein to the saphenous vein graft at one end. Another 3.0 mm coupler was used to anastomose the other end of the saphenous graft to the superficial inferior epigastric artery and vein in order to facilitate venous outflow and channel it from the superficial system into the deep system.

Surgeon said he done a 35681. I think that is incorrect. 35681 is a composite bypass graft. And that is not what was done.

I was thinking more along the lines of 35251? Repair with vein graft includes harvesting correct? Anyone have any thoughts?
