Hi. I have a question and am wondering if someone out there has some guidance? Our Gyn Oncologist had a visit with a patient that has a history of ovarian cancer. Patient had a hysterectomy years ago. Isn't it more appropriate to code this as an office visit versus a preventive visit? Please let me know what you think.

If a current problem is being addressed, then use problem oriented. If the history is just mentioned, then it's just that - history. It would be included in the comprehensive history portion of a preventive exam if it's just a routine checkup.
