Wiki Wellcare State Rate Sheet

Does anyone know the correct way to create a state rate sheet for Wellcare? I'm having trouble with my submission. Please help! Thank you.

I have just come across that problem with Wellcare. I was told that there is a form in CMS. I did a search in the CMS website but could not find it. Were you able to get it? If so, how did you do it?
