Hello, I need to know what POS falls under the CPT Q3014. Been having a lot of denials for this code. Also, The rendering provider is the site itself. Should I change it to the provider that 's rendering the services??? Any help would be greatly appreciated. molly.staley@icloud.com Thanks in advance!
 
Q3014 is a facility fee that is excluded from coverage on the physician fee schedule because the code was created specifically for use by facilities billing on a UB-04. Under normal circumstance, this code would not be payable with any POS if billed by a professional provider.

I have read that payers may be allowing this as an exception under the current COVID health emergency, but I have not seen any published coding guidance that that says how this should be billed by non-facility providers. You may need to contact the payers who are denying this to find out if they are in fact allowing it to be billed by non-institutional providers and if so, what place of service they require for payment.

The originating site facility can bill Q3014 (originating site facility fee) and that can be submitted on the CMS 1500 or UB-04."
If the provider clinic is using staff to facilitate an originating site connection for a patient in the office with a specialist. The clinic physician is not even present in the fee-for-service clinic at the time. This is now billable by the clinic on the 1500. What names/NPI should go on the claim in J: rendering provider ID #, 32. Service Facility location info and 33. Billing provider info?
