When billing bilaterally on one line, are we supposed to double the billed fee?

Metaphorical example, if we are billing a line at $400.00 and the service reimburses/allows from insurance at $350.00 for one unit, and then the bilateral portion of the CPT would be paid/allowed half at $175.00.

Example 1
CPT Units Billed Amt Reimbursed Amt
XXXXX-50 1 $400.00 $400.00

Example 2
CPT Units Billed Amt Reimbursed Amt
XXXXX-50 1 $800.00 $475.00 ($350+$150)

In above example, is 1 or 2 correct? Does anyone have any articles pertaining to this?

When/Is below example appropriate?
Example 3
CPT Units Billed Amt Reimbursed Amt
XXXXX-50 2 $800.00 $475.00
