Can I please get claification on when it is appropriate to bill 97001 (physical therapy evaluation) vs 97002 (physical therapy re-evaluation)? We have a patient that was doing fine and was discharged from physical therapy, and six months later is having the same problem again. Can we bill a new evaluation or, since its the same, site is it a re-eval?
