NP saw patient for cellulitis and abscess of wound..illeal conduit. She wants to bill 97602 wound care..documentation says ostomy present draining clear urine with appliance intact, stoma is pink and moist, there is what appears to be a ureteral stent passing through the ostomy, surgical staples in place, incision appears to be healing well except about one inch distal to the most superior aspect of the incision where the inscision has opened, immediately lateral to this area is 2 inch horizontal opening in the skin to the subcu tissue, this area is draining malodorous green, thick, purulent drainage, surrounding erythema and induration has decreased but is still present. Not sure if this note justifies 97602...Any help would be appreciated!
 
The description of the CPT states "debridement of wound....wet on wet...." - by the information you provided, it does not appear that any actual procedure was done. Can you elaborate for us?
