POSITION OBJECTIVE

The Coding/Documentation Specialist assists with both the ACO and the Dual-Eligible projects. This position will be responsible for performing educational and implementation activities that support the clinical, business and program related work with a strong focus on RAF and HCC coding and documentation.   

MAJOR RESPONSIBILITIES

·      Strong Coding and clinical documentation experience

·      Lead the annual CMS GPRO audit focusing on quality measures

·   Provide education and resources to ensure medical practices have the knowledge needed to succeed.  

·  Responsible for participation in projects and/or assignments including, but not limited to strategy initiatives, both interdepartmental and external projects

·  Work collaboratively to complete annual GPRO audit which includes appointment setting, assistance with data gathering and submission to CMS portal

· Work collaboratively to review Dual-Eligible participant performance within project to develop practice-specific strategies which support driving metrics outcomes

·      Schedule, prepare and lead meetings with practices focused on documentation, coding, quality performance gap closure and develop and implement follow up activity

·        Work collaborative with practice engagement initiatives

·        Leads or provides education/training to clincal care (physicians, nurses, etc) and administrative support team members regarding the clinical documentation requirements

·     Coach practices around workflow to support achieving metrics thresholds

Associate or Bachelor’s Degree in Health Information Management (preferred)

Must possess a valid driver’s license in the state of California and provide proof of insurance

Dexterity of hands and fingers to operate a computer keyboard, mouse and other devices and objects.  Physically able to sit for long periods of time.

Automobile travel within Riverside and San Bernardino Counties
