Level 2 is responsible for accurate and timely coding and abstracting of
inpatient, day program, and outpatient facility records for the Shepherd Center
including Pathways and the Shepherd Pain Institute. Reviews documentation in
inpatient and/or hospital outpatient/clinic medical records, and accurately and
completely assigns diagnostic and procedural ICD-10-CM, ICD-10-PCS, and/or
CPT-4/HCPCS codes according to established coding guidelines and industry
standards. Abstracts demographic and coding information into the document
management and/or electronic health record system accurately and completely.
Works closely with medical staff and other clinical staff/departments to ensure
the accuracy and completeness of clinical documentation. Participates in compliance
audits and related coding training/education activities as required.
• High school graduate required;
Associate's and/or higher degree preferred.
A minimum of
3 years experience coding inpatient and outpatient facility records in an acute
care hospital, healthcare system, or large physician practice.
• Long-term, acute hospital experience
preferred but not required.
• Must have and maintain a 95% or
higher accuracy in abstracting, coding, and DRG and/or APC assignment while
meeting established productivity requirements.
• Good computer skills with experience
in computerized encoding and abstracting systems in a Windows based
• RHIT, RHIA, CCS and/or CPC required.
• Knowledgeable and proficient in
Microsoft Office applications.
• Computer skills with experience in
computerized encoding and abstracting in a Windows based environment.
• 3M and Epic experience preferred
