Responsibilities include:

·         Reviewing Inpatient/Long Term Care/Outpatient records to ensure appropriate ICD-10-CM/PCS codes are accurately assigned by coding staff.

·         Identifying trends and opportunities for improvement in clinical documentation.

·         Collaborating with coding professionals to fully support the needs of clinical code assignment, and communicating proficiently with coding team to resolve discrepancies.

·         Identifying co-morbidities and complications, as well as documents appropriately.

·          Querying medical staff and other clinical caregivers as necessary to obtain accurate and complete documentation.

·         Identifying potential quality, severity of illness, risk of mortality, hospital/physician profiling, and reimbursement issues or missing documentation.

·         Interacting with the coding team as documentation issues are identified through the coding process for discussion with clinical staff.

·         Providing coding support to Inpatient/Out-Patient Coders when required.

·         Handling other related functions as necessary.


Qualifications include:

·         High School Diploma or equivalent. College preferred.

·         Coding Credential from AHIMA/AAPC (RHIA/RHIT/CCS) as Certified Documentation Specialist (CCDS)

·         3 years of inpatient medical coding experience

·         Prior ICD-10 experience and familiarity with 3M systems

·         Knowledge and understanding of official physician E&M guidelines and documentation requirements in support of proper E&M assignment and establishment of medical necessity.
