The Health Risk Assessment Program supports UCLA primary
care in ongoing proactive, complex care management of high risk patients to
provide a comprehensive management approach to primary care. The Health Risk
Assessment program serves as an extension of the patient's primary care team.
The Supervisor will be the lead role in development and management of the
program, which will include innovative approaches to care to enhance UCLA's current
care model in primary care. The key goals of the Health Risk Assessment program
are to ensure accurate risk adjustment and chronic condition documentation,
coding, and monitoring of chronic conditions. The primary functions will be
largely conducted remotely through pre-visit chart assessments to support
comprehensive primary care visits. The program may include in-home visits to
high risk patients based upon clinical needs, as per the direction of the UCLA
primary care team.
The Supervisor will apply advanced clinical skill and
expertise in history taking, physical exam, treatment and clinical care
management of patients. The Supervisor and the Health Risk Assessment Program
staff will monitor, train and coach patients on how to live with chronic conditions
to ensure health goals are met. The Supervisor will direct administrative,
clinical and fiscal operation of Health Risk Assessment Program.
CA RN and NP license required
National Certification as an NP
BLS certification from the American Red Cross or American Heart Association
Master's degree or Doctoral Degree in Nursing, required
Certifications: State of California NP,
State of CA NP Furnishing with Schedule II, National Certification (i.e.
Enrollment in PECOS
Enrollment in CURES
3-5 years managed care diagnostic coding or billing related experience required
Understanding of HCC & ICD-10 Coding highly desired
