To be fully engaged in providing Quality/No Harm, Customer Experience, and Stewardship by data collection of important processes and services, identifying changes that enhance performance, and maintaining a continuous level of patient safety, and accreditation readiness with regulatory agencies.
Primary Accountabilities
- Proactively seek opportunities to assist with the identification of risks to patient and associate safety. (Analyze Patient Safety Portal Reports)
- Establish necessary action plans in collaboration with management team to improve documentation and patient safety.
- Supports the Agency’s mandatory quality initiatives. Conduct National Patient Safety Goal clinical record reviews and submits data to the Outpatient and Wellness Dashboard. Submits monthly PowerPoint presentation of NPSG to Quality Council including any necessary action plans for patient safety and improving quality.
- Coordinate the indicator monitoring and performance measurement for the Agency. Conduct Additional Development Requested, and Face to Face documentation audits for CMS to ensure adherence to the Conditions of Participation for Home Health Care.
- Monitor performance through data collection. Conduct active and discharge clinical record reviews with follow through to appropriate supervisor/manager to improve quality and adherence to regulatory requirements. Conduct clinical record review for patient concerns or complaints as assigned.
- Analyze current performance. Submits supervisory field visit data and infection control data to Infection Control team monthly.
- Responsible for the accurateness and completeness of the data.
- Assists with Joint Commission readiness on an ongoing basis.
- Performs case review, as assigned, with staff on a regular basis to ensure appropriate utilization. Assist Clinical Educator with OASIS training for clinical field staff.
- Assist Clinical Educator with clinical staff education concerning adherence to Florida Statues, AHCA, Medicare guidelines, Joint Commission in regards to Home Health Care.
- Collect and abstract patient information in the medical record using clinical judgment and knowledge of clinical systems.
- Use required software and collection tools.
- Maintain reporting deadlines.
- Provide reports (from clinical record review) to committees or groups as requested.
- Review data to assess for developing trends.
- Submit Quarterly AHCA reports.
- Export OASIS Assessments at least weekly, obtain OASIS data quarterly for review.
- Attend committee meetings as assigned.
- Assists in developing action plans for those areas not meeting Patient Satisfaction as requested.
- Performs field supervisory visits as part of Joint Commission readiness preparation for clinical field staff
- Works with billing department to ensure proper documentation guidelines are followed for appropriate reimbursement.
- Works with the Performance Improvement Supervisors to ensure appropriate utilization.
