Venipuncture useage

We are wondering how to code a glucose test (82962) in office. Do we code 89262 with or without (36415) venipuncture?

Also when drawing in office for INR, we should be using the venipuncture code, is that correct?

Thanks for any assistance you can give us on this.

I have worked in many different offices as well as for a major health insurance company (18 years). It is always appropriate (in my experience) to bill for a venipuncture when blood is being drawn in the office.
