I have a cardiology coding question regarding Transesophageal Echocardiography (TEE). Often we see that some of our cardiologists do a pre and post surgical TEE. How can I bill for both visits?

Sometimes the same physician performs both pre and post surgical TEE and*sometimes two different physicians perform this exam.

Can some on explain me in brief for my following queries:

1)If both pre and post TEE's can be billed?
    2)Are they both payable?
3)If same physician does what are the codes and modifier i should use?
4)If different physician does what are the codes and modifier i should use?
5)What is the coding guidelines for this procedure?

  2. Default Tee's
    Hi Shirley,

A few of our cardiologists perform TEE's, both pre and post operatively;however we do not always receive payment on the post op TEE.

Some payers will pay the post op TEE, Medicare usually will not pay. To bill for a pre and post op TEE: 93312-26, 93320-26 & 93325-26. post op you would use the same CPT codes but will add modifier 58. If two physicians perform the TEE, for the post op TEE you would add modifers 26 & 76 indicating this is a repeated procedure but different doctor.

    Good luck,
