HPI - who can document

  1. Default HPI - who can document
    Medical Coding Books
    A discussion has come up at our clinic as far as who can document the HPI and have it count. I believe that it has to be the Physician/PA who doucments the HPI in order for it to count. Is this correct?

    Please chime in!!
