Medicare Colonoscopy

    Medical Coding Books
    If I am billing a screening colonoscopy to Medicare with the patient having a history of polyps would I bill G0105 or G0121? Also, would I use both V76.51 and V12.72 as my dx's or just V12.72? If I do use both which would be my primary dx?
