    Medical Coding Books
    Question. (Medicare).

    When a physcian refers a patient for P.T services. And the P.T services are rendered. What is required from the P.T in the documentation, pertaining to Medicare. I see you need a 30,60, 90 day certification. I could be reading this wrong educate me on this.

    What does medicare look for in regards to this 30,60,90 day certification.

    The therapist must sign the certification within 24 hours for it to be legitimate or you cannot bill the patient for the new certification period. It also must include the plan of care.

    Explain more please.

    Educate me.

    What is this certification. Does the PCP give this to the patient prior to the patient receiving PT services?

    Is there a code for this certification.

    If you could, can you give me a scenario for this. In regards to Medicare patients.

    Try this It can help you tremendously as it is the American Physical Therapy Association specifically for P.T.'s.

    Default PT
    It is my understanding that they require time in/out, dx code, body part and side of body, modalities performed, plan of care and exercise log.
