    For example:

    Date of visit 2/3/2018:

    Initial patient care in hospital - detailed history, comprehensive exam and moderate MDM - 99221
    Diagnosis codes from Subject and A/P:
    R10.0 – Patient has acute abdominal pain
    R11.2 – Nausea with vomiting, unspecified
    J44.9 – Chronic Obstructive pulmonary disease, unspecified

    2/4/2018:
    Subsequent care - no complain, feeling better - 99232?
    Subject and A/P same as above. What codes for diagnosis?
