    • Painful Achilles tendinosis, right heel.
    • Painful bone spur, right calcaneus.

    • Excision of inflamed tendon with repair of right Achilles tendon.
    • Excision of bone spur, right calcaneus.
Attention was directed to the lateral and posterior aspect of the patient's right heel, where an incision was made just lateral to the Achilles tendon insertion on the patient's posterior heel. The incision was deepened through a mixture of sharp and blunt dissection, down to the level of the calcaneus. Next, a flap was lifted, including the lateral portion of the Achilles tendon insertion, down to expose the posterior lateral and posterior aspect of the patient's calcaneus. A loose bone spur was encountered within the substance of the Achilles tendon and partially attached to the calcaneus. Using an osteotome, mallet and a bone rongeur, the bone spur was completely removed, and removed from the tendon. At this point, the tendon was evaluated. Inflamed portions of tendon were débrided out and placed on the back table. The surgical site was then copiously irrigated with normal saline. All remaining bone spurring and prominent posterior calcaneus was removed using a bone rongeur. The area was then copiously irrigated with normal saline. The Achilles tendon was repaired using 2-0 Vicryl. The skin was then closed using 4-0 Vicryl and 4-0 nylon after bleeders were cauterized.
