    Good Morning,

    Patient underwent base wedge osteotomy 5/5/09 at our ASC. Postoperatively,
    the screw had backed out of the osteotomy indicating it had failed to
    compress the osteotomy. The pt was brought back to the operating room
    yesterday for removal of screw and placement of a longer screw. This is
    from the op note. Thanks in advance for your help.

    "Attention was directed to the dorsal aspect of the previous incision site.
    The sutures were still in place. About one half of the length of the
    incision was opened proximally, exposing the base of the first metatarsal.
    The fixation devices were identified and left in place temporarily. The
    C-arm was used to locate the position for the new screw. We decided to place
    a larger screw a bit more proximally in the osteotomy. The C-arm was used to
    guide the drill. The area was measured, counter sunk and a 3.5 mm screw was
    placed across the osteotomy and tightened. Better compression was achieved.
    The remaining earlier placed fixation devices were then removed. The 2.7 mm
    screw was removed and replaced with a 3.5 mm screw"

    I would use unlisted code because there is no code for removal and replacement or just replacement of a screw.
