    I'm having trouble deciding which code to use for the following report. I can't decide between 78460 78461 78464 78465. I know to code 78478 and 78480. He is not stating planar or SPECT. What else is missing from his documentation? Thank you very much.
    (I'm billing for a cardiologist so I also code 93016 and 93018 - done in a hospital)

    Pharmacological stress test using IV Adenosine. Myocardial perfusion study was performed using dual isotopes. THe study is consistnet with perfusion defect in the left ventricular inferior wall suggestive of myocardial scar. THere is an area of stress-induced ischemia in the same territory. The size of ischemia is small, but it is of at least moderate intensity. LVEF by gated study is computed at 54%

    I think based on what you have listed above, I would go with the 78460 ( Myocardial perfusion imaging: single study. )
