    Default three codes or two?

    AAPC: Back to School
    One large polyp biopsies and tattoed with India ink(proximal sigmoid) AND two polyps excised by hot snare(rectal): 45380/45381-59/45385???

    Looks like 3 to me. I don't think you need Mod 59 on 45381, but on 45380 instead.

    I agree....3 codes, "59" on 45380.
