Critical Care and modifier usage

    If critical care is performed for 90min and the decision to insert a catheter was made---Would you use the -57 mod on JUST the 99291 or on both the 99291 and the 99291?

    -57 is use with a 90 day global
    I would use -25 on 99291 and 99292
