    Medical Coding Books
    If I have a PE, E/M and Procedure on one Visit. I would put a 25 Modifier on the E/M but does the procedure need a modifier and if so what one?

    No. To give an example, this how it would apply


    CPT 99396
    CPT 9921X. 25
    CPT 17110



    No modifier needed on the procedure, unless multiple procedure come in to play.

    In this case you need a 25 on both E/M codes.

    99396-25
    17110
    9921x-25

    The 99396 will bundle into the procedure without the modifier on it.
