    AAPC: Back to School
    Hello, was wondering if the response "noncontributory" in re hx on e&m coding is thought of and allowed as ..gone over or???

    I don't allow this statement nor does my Medicare carrier...


    Better way:

    Patient denies any pertinent XXXXXX (family history) associated with the current problem" (Example only)

    Default Trauma

    I had a surgeon ask how there could be any relevant family medical history to a trauma. My response... depending on your treatment plan you might want to know if there is any family history of bleeding disorders or difficulty with anesthesia. Two quick and easy questions that are medically relevant to your treatment of the problem, and satisfy the documentation requirements as well.
