    Default Removal Instrumentation

    AAPC: Back to School
    Any feedback would be greatly appreciated...

    Operation Performed:
    L2-L3-L4 Decompression
    Removal L4-L5 instrumentation

    My physician billed 63047, 63048 X 2 and 22852

    The 22852 (instrumention removal) is bundling with 63047. Is it appropriate to add a 59 modifier?

    Yes, as long as it is a different vertebral level. You need to mark that in the "narrative" section of the claim.
