Patient came in for a work PE - he has DX of 250.02, 414.01, 401.9 and 296.80. In terms of ICD-9 coding for the visit, would you use V70.5 as primary and then all associated DX as secondary codes or only use the V70.5? thanks :-)

Use the V70.5, use the associated dx as secondary only if they were managed, controlled, or treated at this encounter.
